Subhepatic perforated subhepatic appendicitis versus acute cholecystitis: a diagnostic dilemma

Appendicitis, being one of the most common surgical problems, is often diagnosed clinically. The typical clinical sign elicited from a patient suffering acute or perforated appendicitis includes: Rebound tenderness, Rovsing sign, Obturator Psoas and tenderness over right iliac fossa. Cholecystitis, another problem, typically presents with hypochondrium, associated this condition positive Murphy’s sign. In report, we describe how subhepatic can mimic cholecytstitis, both clinically radiologically in an adult patient.

A case of acute acalculous perforated cholecystitis with acute generalized peritonitis in a middle aged cachectic man, presenting late in a moribund condition, is reported. He underwent emergency laparotomy (subtotal cholecystectomy), went into multi-organ failure and was managed accordingly. The patient recovered in about 2 weeks and was followed-up regularly.
